Residents in Thurrock have been reminded they can access support if they have had to undertake a spell in hospital and are recovering at home.

Keeping warm, collecting medication, making sure there is food in the fridge, and having the means to do all of these things is important after a stay in hospital.

The Mid and South Essex Integrated Care System, a partnership between organisations that meet health and care needs across the wider region, has issued a briefing.

It says: "If you or someone you know needs help, support is available from local community hubs.

"The community hubs can help connect you into support that meets your needs in your local community.

"This can be everything from support with money management, mental health support, getting more active and parenting support."
