Have you received hospital care - and what happened after you were discharged?

CONCERNS about the outcomes of the discharge of borough residents from acute patient treatment within the Mid and South Essex NHS Trust and beyond have prompted a call for more information from patients and professionals by Thurrock Healthwatch, the independent champion for people who use health and social care services in Thurrock.

Healthwatch wants to hear the personal experiences of patients who have received acute treatment and those of their carers and family members.

They particularly want to hear what has happened following treatment at Basildon, Southend and Chelmsford hospitals - but experiences of Thurrock people at other hospitals will also be welcomed.

Patients, carers and families that have a story to tell are asked to record it by this survey: http://tinyurl.com/yadzkumr.

And the service is also reaching out to professionals involved along the way with all aspects of patient care at local hospitals and clinics, particularly those related to the current discharge pathway.

They are invited to complete this survey: http://tinyurl.com/4z6sv4ja

A spokesperson for Healthwatch said: "Our aim is to capture the lived experience of patients to inform opportunities for service transformation and change – if people would rather give us a call to discuss their experience they are more than welcome to do so on 01375389883.
